We will only fulfill requests for personal information associated with the personal information we store about you that is connected with the identifying information you provide. For instance, if you believe your personal information may be associated with more than one email address, you must submit a separate request for each email address and separately verify your identity. We will fulfill verified requests within a timeframe consistent with the law of your state.

If you are an authorized agent submitting a request on behalf of another individual, please provide the information for both your client and yourself.

NOTE: When we handle information about job seekers, we do so on behalf of the employer as a service provider or processor. This form does not apply to information collected by employers through direct engagements. If you are submitting a request for seeker information collected through an employer direct engagement, please contact the employer and we will await their instructions on how to handle it.
